Is there an item out there that gives you a chunk of bonus to a skill check, like heal?

Lots of items are already in the game that do things like that, but the custom skill bonus item is the most commonly accepted sort of custom item.

The formula is skill bonus x skill bonus x 100gp.

So an item of +5 Heal (like a libram or a ring or boots or cloak or enchanted surgeon's tools) would cost 5 x 5 x 100, so 2500gp.

This bonus is always a competence bonus, if you want a different kind of bonus the cost is either x5 or x2, I forget which.

Note also that this is the cost for an item that occupies a slot. Items that do not occupy a slot (such as the aforementioned surgeon's tools) will cost more. I believe the modifier is x2.

1.) Is there a comprehensive item guide?
2.) Would a custom item graft count as a custom/no slot item?

The DMG has guidelines on pricing magic items, yes.

As for a comprehensive item guide, the Magic Item Compendium is probably your best bet as far as a physical sourcebook goes.

Custom item grafts are, naturally, custom items...but graft rules are slightly different, either limiting a person to having a single graft or to having a single graft on a very limited number of body positions, depending on which books you're looking through. Reasonable limitations are one graft on your: head, back, legs, arms and torso/heart.

The bonuses are untyped, so yes, if you have both items you increase your Push (but ONLY Push) distance by 2 squares. Had the bonuses been the same type, like an Item bonus, for example, they would not stack.
